Report a Concern

Report a Concern

Health & Safety • Accidents • Safeguarding

Your safety, and the safety of everyone in our church community, matters.
This page is the main place to report concerns about:

  • Health and safety issues

  • Accidents or near misses

  • Safeguarding concerns (children, young people, or adults at risk)

Reporting helps us respond quickly, support those affected, and prevent future harm.

If Someone Is in Immediate Danger

If anyone is at immediate risk of harm:

  • Call 999 straight away

  • Then report the incident using the appropriate form, using the links below, when it is safe to do so

What Should I Report?

Please report anything that has caused harm or could cause harm, even if you’re unsure.

Health & Safety

Examples include:

  • Unsafe buildings, equipment, or trip hazards

  • Fire safety concerns

  • Broken furniture or electrical issues

  • Anything that feels unsafe

Accidents & Near Misses

Examples include:

  • Injuries to adults or children

  • First aid incidents

  • Slips, trips, or falls

  • Near misses (something that almost caused an injury)

Safeguarding Concerns

Examples include:

  • A concern about a child, young person, or adult at risk

  • Something you’ve seen, heard, or been told

  • Behaviour that feels worrying or inappropriate

  • A disclosure made to you

You do not need proof. It’s okay to report a concern or a gut feeling — it’s better to share than to stay silent.

How to Report

Please choose the option that best fits your concern:

Each form will ask for:

  • What happened

  • When and where it happened

  • Who was involved (if known)

  • Any immediate actions taken

You can share as much or as little as you know.

Confidentiality & Respect

  • Information is treated sensitively and seriously

  • Safeguarding concerns are only shared with those who need to know

  • We follow national safeguarding guidance and legal requirements

  • Your role is to report, not to investigate

What Happens Next?

Once a report is submitted:

  1. It is reviewed by the appropriate responsible person

  2. Immediate action is taken if needed

  3. Further steps are agreed in line with our policies

  4. Records are kept securely

Where appropriate, we will follow up for clarification or to offer support.

Need Help or Unsure What to Do?

If you’re not sure which form to use or need advice:

  • Speak to a member of the church leadership

  • Contact the Safeguarding Lead or Health & Safety Lead
